All access to the flat files of the MIMIC II Clinical Database is via PhysioNetWorks, so begin by creating a PhysioNetWorks account if you have not already done so.
Once you have done so, request access (either from the link at the end of this paragraph, or from the one on your PhysioNetWorks home page). You will need to take an approved human subjects protections training program and submit a certificate of completion from it, together with a MIMIC II Restricted Data Use Agreement (DUA); full details are available here.
All requests are reviewed, and if yours is approved you will receive an email containing instructions for accessing the data via PhysioNetWorks. Note that approval usually requires one or two working days, and will be delayed if your request is missing any required information.
